Transitional Services
Youth in transition from a child system to a more independent adult system require supports and guidance to understand the differences, correctly access the care they require and inform the system of their adult system with their needs and desires.
Clients that are transitioning from treatment with the Child and Family Network and moving toward treatment at our Adult Outpatient Network. Using a Wraparound model, services assist clients in addressing the following domains: Employment and Career, Community Life Functioning, Educational Opportunities, and Living Situation.
Eligibility Criteria:
The primary clinician at the Child and Family Network completes a referral to the transitional group and works with the Adult Network to follow client through the process.

Medicaid, Medicare, Self-Pay, Block Grant and Third Party Insurances
Hours
Monday-Friday 8:00 a.m. to 5:00 p.m.
